Cataract Operative Reports: Standard vs. Complex

The following are examples of the operative reports you should receive for cataract patients. Each report indicates whether the patient underwent a standard cataract extraction or a complex cataract extraction, identified by the corresponding CPT code. In order to receive proper reimbursement from insurance companies, you should be sure the insurance code you bill matches the procedure listed on the operative report:
