This sweepstakes is governed by these Official Rules (“Official Rules”). By participating in the Sweepstakes, each entrant agrees to abide by these Official Rules, including all eligibility requirements and understands that the results of the sweepstakes, as determined by Sponsor and its agents, are final in all respects. The Sweepstakes is subject to all federal, state and local laws and regulations and is void where prohibited by law. ELIGIBILITY
The Sweepstakes is open to legal residents of the 8 county region of Western New York, who are eighteen (18) years of age or older at the time of entry. Sponsor has the right to verify the eligibility of each entrant.
Employees (including, without limitation, part-time or temporary employees) of Fichte Endl & Elmer Eyecare and contest sponsors their respective parent entities, subsidiaries, affiliated companies and advertising and promotion agencies and the immediate family (i.e., current and ex-spouses, parents, grandparents, children, grandchildren, and siblings and in-laws and steps in any of the foregoing categories) and other household members (i.e., roommates, housemates, significant others, and partners of each legally residing at the same address) of each are NOT eligible.
SWEEPSTAKES PERIOD. The Sweepstakes begins at the start of the Laser Vision Night on Thursday, March 14, 2024, at 5:30 PM, and ends at the conclusion of the May 2024 Laser Vision Night (“Sweepstakes Period”). All entries (submissions) must be received on or before the time stated during that submission period.
HOW TO ENTER. Enter the Sweepstakes by attending one of our LASIK Open House events during the months of March, April, or May 2024. Entrants will submit an entry form in person while attending a Laser Vision Night in either March, April, or May 2024. We will be offering one Free Laser Vision Correction Procedure. You may only enter ONE of these sweepstakes in March, April, or May 2024.
WINNER SELECTION. All eligible entries received during the Submission Period will be gathered into a database and the winner will be randomly selected at the conclusion of the May 2024 Laser Vision Night. The winner will be contacted by telephone by representatives of Fichte Endl & Elmer Eyecare by May 31, 2024. One (1) winner in total. The entrant is responsible for monitoring his/her phone for prize notification and receipt or other communications related to this contest. If a potential prize winner cannot be reached by the Administrator (or Sponsor) within two (2) days using the contact information provided at the time of entry, the prize will be forfeited and an alternate winner will be selected. Upon the request of the Sponsor, the potential winner may be required to return an Affidavit of Eligibility, Release and Prize Acceptance Form and IRS W-9 form. If a potential winner fails to comply with these official rules, that potential winner will be disqualified.f
PRIZE. One (1) FREE Refractive surgery/LASIK procedure. Retail value up to $4,200. One (1) will be given away in total. Not redeemable for cash and/or credit. The prize IS transferable. This means if the winner is not a surgical candidate or was entering to win it for another person it may be used on someone other than themselves. Before surgery can take place the winner must have a LASIK evaluation performed by the doctors at Fichte Endl & Elmer Eyecare to deem them an appropriate candidate. Winner must meet all surgical criteria for surgical eligibility.
OTHER TERMS:
All follow up appointments’ must be kept after surgery.
The surgical candidate will be expected to sign consent forms.
No claims or guarantee of a 20/20 vision outcome after surgery.
Fichte Endl & Elmer Eyecare reserves the right to amend or add rules to this sweepstakes at any time.
